Flu Drugs Causing Bizarre Behavior?

(AP) Government health regulators recommended adding label precautions about neurological problems 151; including hallucinations and convulsions - seen in children who have taken flu drugs made by Roche and GlaxoSmithKline.

The Food and Drug Administration on Friday released its safety review of Roche's Tamiflu and Glaxo's Relenza. Next week, an outside group of pediatric experts is scheduled to review the safety of several such drugs when used in children.

FDA began reviewing Tamiflu's safety in 2005 after receiving reports of children experiencing neurological problems, including hallucinations and convulsions.

Twenty-five patients under age 21 have died while taking the drug, most of them in Japan. Five deaths resulted from children "falling from windows or balconies or running into traffic."

There have been no child deaths connected with Relenza, but regulators said children taking the drug have shown similar neurological problems.

While FDA said it isn't clear whether the problems are directly related to the drugs, it recommends adding language about the possible side effects to labeling for physicians who prescribe Tamiflu and Relenza.

Besides being a drug <side> effect, the agency said the behaviors alternately could result from an unusual strain of flu or a rare genetic reaction to the drug.

Company representatives were not immediately available for comment.

A new report from Trust for America's Health found that over half a million Americans could die and over 2.3 million could be hospitalized if a moderately severe strain of a pandemic flu virus hits the U.S. Additionally, based on the model estimates, 66.9 million Americans are at risk of contracting the disease. TFAH's report, "A Killer Flu? ‘Inevitable' Epidemic Could Kill Millions" also examines shortfalls in the amount of available antiviral medication and hospital capacity to respond to an epidemic. Additionally, the report examines gaps in U.S. readiness and compares U.S. preparedness to the United Kingdom and Canadian efforts. TFAH offers detailed recommendations to help improve U.S. preparedness.

TFAH provides a series of detailed recommendations to help ensure the U.S. is better prepared regardless of whether a pandemic occurs as soon as this year or in several years. With a crisis looming, the US plan for the pandemic should be finalized and the President should be designated an official with authority to coordinate the US response across federal agencies. Other top level recommendations include taking:

TFAH's analysis, based on the estimates of the severity of the current strain circulating in Asia, follows warnings issued by the Centers for Disease Control and Prevention (CDC) and the World Health Organization (WHO) about the severity of the pandemic, "killer flu" threat. The projections are based on a modeling program developed by the CDC using WHO estimates that approximately 25 percent of countries' populations could become infected and descriptions of the severity of the strain as likely to be in the range between the levels of the extremely severe 1918 influenza pandemic and the relatively mild 1968 pandemic. Some scientists believe the current avian flu strain is on the more severe side of the possible range. The more mild and more severe estimates are also included in the appendix of TFAH's report.

Since the original broadcast of this program, Taubenberger's team has successfully created a genetic sequencing of the 1918 virus, resurrected the virus itself to study its effects on lung tissue, and this fall announced a striking similarity between the 1918 virus and today's H5N1 avian flu virus. Their findings indicate that the 1918 virus originated as a bird flu, confirming the legitimacy of concerns about avian flu. The updated episode includes new material and interviews with Taubenberger that reflect these new findings.

Over the centuries, humans have endured many influenza pandemics. Descriptions of the disease in Europe date back to 1100 A.D., and sporadic, isolated outbreaks have likely been occurring for many thousands of years. But no flu ever struck as fast, as hard, and with such lethal power as the 1918, or "Spanish Flu," which rivals the Black Death as the deadliest epidemic in history. To a world already ravaged by war, the 1918 pandemic was crippling; some 30 to 40 million people died worldwide, with the highest death rate occuring in young men and women. In the United States, 675,000 people died, including over 40,000 G.I.s 40 percent of all of those who perished in World War I.

A US team added two genes from a sample of the 1918 virus to a modern strain known to have no effect on mice.

Animals exposed to this composite were dying within days of symptoms similar to those found in human victims of the 1918 pandemic.

They believe the virus, which claimed the lives of up to 50million people around the world, jumped from birds to humans.

The breakthrough, published in Science, should help doctors identify which future bird viruses pose a threat to man at an earlier stage.

But the National Institute for Medical Research team warns viruses cannot be stopped from crossing between species.

During the last 50 years, hundreds of thousands of military personnel have been involved in human experimentation and other intentional exposures conducted by the Department of Defense (DOD), often without a service member's knowledge or consent.

The military has released chemicals and biological agents through outdoor "open air" tests for over four decades. Some of these supposedly safe chemicals and biological agents, referred to as simulants, were also released over populated areas and cities.

Anthrax vaccine was given to approximately 150,000 military personnel in the Persian Gulf. Anthrax vaccine is considered effective for protecting against anthrax exposure of the skin; however it is unclear whether it provides protection against inhaling aerosolized anthrax. According to the Department of Defense, in biological warfare the anthrax would be sprayed, so the efficacy of the vaccine against aerosolized anthrax would have been the relevant test. Police with Dogs: Vaccinating Kids in Maryland

The State is very clear about medicine: If you want to remain a free citizen, you must submit to the synthetic drugs made by the very same corporations that now control government health regulators. Any person who resists such "treatments" will be branded a threat to public health -- a designation just beneath "terrorist" in the eyes of many government bureaucrats. As such, they believe there is no limit to the level of force they may use to coerce such people into submitting to Big Pharma's chemicals. Today, it's armed guards with attack dogs. Tomorrow, it might be water boarding or other torture methods. Think that's impossible? The "free" America we all once knew is long gone, and it has been replaced with The United States of Corporate America, where police tactics are now used to enforce hazardous public health policies, and the people who run the State no longer think there's anything wrong with rounding up the population at gunpoint and performing large-scale medical experiments on their children. Apparently, the children were being re-vaccinated with not just hepatitis B and chicken pox vaccines, the two new vaccines added to the Maryland school requirement list, but also with other required vaccines for which the public school system could find no record. One mother told me her children were up-to-date on their shots but the school system lost the records and she had to give her children all the required vaccines on the spot or face jail or fines.

My son, Chris, who became learning disabled after suffering a serious reaction to a fourth DPT shot in 1980, traveled with me to Maryland carrying a camera. After growing up watching his Mom work to change one-size-fits-all vaccine policies that were responsible for his vaccine reaction, Chris recently decided he wants to help NVIC put a face on what it means to be vaccine injured in America and what it means when Americans do not have the right to freely exercise informed consent to vaccination.

Chris set up his camera as I talked with a mother hundreds of yards from the front of the Courthouse door. I was about 12 inches inside a row of large cement balls that apparently were erected as a barrier to prevent terrorist attacks. I did not know I wasn’t supposed to be talking with this Mom inside the barrier. She was telling me about how she wasn’t given any information about vaccines before her children were injected with three vaccines.

All of a sudden, out of the corner of my eye I saw an armed guard with a dog emerge from the Courthouse and walk toward us. I got a sick feeling in the pit of my stomach. It was the dread that any citizen of any country in any century has ever felt when an armed guard with a dog starts advancing. As if we were common criminals or terrorists, he yelled and gestured to us to move behind the stones.

We moved without a word. And the sick feeling in the pit of my stomach told me we were being shown the power of the State wielded by that armed guard with the dog, just as parents inside the Courthouse were being shown the power of the State wielded by doctors with syringes.

There has been talk this past week about whether or not U.S. vaccine laws are, indeed, laws or whether they are simply recommendations that do not have the force of law behind them. There is now more than 100 years of case law reinforcing the U.S. Supreme Court decision and the right of states to exercise police power to enforce vaccine laws. The post-911 enactment of the Homeland Security Law, the Model State Health Emergency Powers Act and Bioshield I and II makes it clear that the State will use police power to enforce quarantine or vaccination whenever the State chooses to wield that power.

The method of punishment for not obeying U.S. state vaccine laws is up to the state legislatures which make the laws. Today, many state legislatures have turned over vaccine law-making to unelected government health and education officials, who may enlist state attorneys and judges in the court system to enforce punishments. One of the punishments which many states have chosen when children have not received all state mandated vaccines is to bar children from attending school unless they file and the State approves exemptions to vaccination outlined by the State.

Those parents, who do not vaccinate their children and do not either make arrangements with the State to homeschool them or successfully file a state-approved exemption, are in violation of another state law: truancy laws. Failure to send your child to school in Maryland between the ages of 5 and 16 is a misdemeanor punishable by fines and jail time or both. This is the law which the Maryland government officials moved to enforce when they enlisted the help of State’s Attorney Glenn Ivey (D) and Judge C. Philip Nichols to turn parents of unvaccinated children into criminals.

Following the State of Maryland's threats against parents who refuse to have their children vaccinated, children were herded into a Price George County courthouse being guarded by armed personnel with attack dogs. Inside, the children were forcibly vaccinated, many against their will, under orders from the State Attorney General, various State Judges and the local School Board Director, all of whom illegally conspired to threaten parents with imprisonment if they did not submit their children to vaccinations.

The State of Maryland has now turned to Gestapo tactics to force its medical will upon the People, stripping parents of any right to decide how they wish to protect their own children from infectious disease. Health authorities there have already announced their intent to essentially kidnap parents and throw them in jail, removing them from their children for up to thirty days if they continue to refuse to have their children vaccinated. This will all be conducted at gunpoint, with armed personnel and attack dogs at the ready, making sure nobody steps out of line, and suppressing any attempt at public dissent against the Orwellian vaccination policies.

The entire campaign against these parents is blatantly illegal. There is no law in Maryland requiring the vaccination of children, thus parents who refuse to do so may not be legally charged with violating any law. Instead, Maryland health and school authorities are using Gestapo-like tactics, threatening to charge the parents with child truancy violations, criminalizing them for daring to protect their children from the dangerous chemicals found in vaccines (including thimerosal, a chemical additive containing a neurotoxic form of mercury).

Cold virus chief suspect in AIDS vaccine failure

Researchers halted test after volunteers became more susceptible to virus

WASHINGTON - A cold virus used to make an experimental HIV vaccine that was discontinued in September somehow may have caused volunteers to be more susceptible to the AIDS virus, the vaccine's developers said on Wednesday.

Researchers were doubly dismayed when it appeared that those who had been vaccinated were more likely to become infected, and cautioned more than 3,000 volunteers who had been testing the vaccine that they may be at higher risk of infection.

Merck and the National Institutes of Health agree that the vaccine itself could not possibly cause HIV infection. But initial data appears to indicate that people who got vaccinated had a higher rate of HIV infection that those who got placebo shots.

Experts are hashing out the confusing findings at a meeting in Seattle. Merck and the NIH say that men with the highest pre-existing levels of immunity to a cold virus called adenovirus 5 were the most likely to have also become infected with HIV after being vaccinated.

That adenovirus was modified to use as a so-called vector or delivery mechanism in making the HIV vaccine.

Dr. Keith Gottesdiener, vice president for clinical research at Merck Research Laboratories, said it is possible an earlier infection with adenovirus 5 primed the immune system to produce more of the cells that HIV attacks.

"That is the leading hypothesis — that the vector causes production of more CD4 cells that the virus could infect," Gottesdiener said in a telephone interview.

Attempts to make a vaccine against HIV have flopped because of its unique effects on the immune system. The Merck vaccine was aiming to stimulate production of T-cells called CD8 killer cells, which, it was hoped, would recognize and attack HIV.

Common cold
Usually, vaccines use a whole or weakened virus to stimulate an immune response. This was too dangerous to do with HIV, so the Merck scientists used pieces of genetic material from the virus instead.

To carry them into the body, they used a virus that infects people easily — in this case, adenovirus 5, one of dozens of common cold viruses.

To test the vaccine, the researchers sought volunteers who had a high risk of HIV infection anyway, (useless eaters - they're gonna die anyway) such as injecting drug users and men who have sex with men. The volunteers are counseled on ways to protect themselves, such as by using condoms, but some get infected anyway.

When they combed through the numbers, the researchers found that mostly men who had sex with other men became infected after vaccination.

They also noticed that people with more pre-existing adenovirus 5 immunity — meaning they had been infected in the past and mounted a strong immune response — were also more likely to become HIV infected after receiving the vaccine.

"Among the 778 male volunteers who had high levels of pre-existing immunity to adenovirus 5, 21 cases of HIV infection were observed in those who had received vaccine and nine cases of HIV infection were observed in the volunteers who had received placebo," Merck said in a statement.

But it was possible that people who happened to have high levels of adenovirus 5 immunity just behaved differently, having more risky sex, for instance, Gottesdiener said.

"It could be due to some biological phenomenon that we don't even understand yet or, honestly, it still could be due to chance," he said.

The trial, which began in 2004, had enrolled volunteers in the United States, Peru, Brazil, Dominican Republic, Haiti, Jamaica and Australia. A second trial had begun in South Africa earlier this year with 800 volunteers.

An article came out in Scientific American in October 1995 that was very fascinating reading. They have to deal with the results of this biological warfare, because too many people are getting sick and too many people are dying. How are they going to deal with it? The title of the article in Scientific American was, "Dangers from new viral plagues." Have you been seeing anything in the newspaper about Dengue fever in South America, or about these "strange" viral hemorrhagic diseases that are " suddenly attacking us" from " nowhere "? Guess what. They came out of the Gulf War! And, they are now calling it "emerging viruses". Hemorrhagic fever viruses are among the most dangerous biological agents known. The Ebola virus. You didn't hear about that before the Gulf War, did you?

Why am I concerned about this? I was having lunch with Peter Kawaja, and I noticed this article in USA Today, and I couldn't believe what I was seeing. Remember when there was that period of incredible heatwaves in Chicago and all these people were dying? Well, look here in this article, where it shows lines of refrigerated trucks brought in to store bodies. Since when did we have so many bodies that we had to put them in trucks? This is the part that concerns me: The bodies were stored in nine donated refrigerated trucks that were to be destroyed after use. Now, if we went and destroyed every hospital and funeral home after a body was in there, we wouldn't have much left, would we? Why would they want to destroy the trucks? Were they contaminated? Was there a disease? I don't know. All I know is that when you read material in the newspaper, check it out very carefully. Ask questions! Don't assume they are telling you the whole story.

Peter Kawaja has been talking about this. Guess what. This just came out on 10/10/95. There is such a thing as sexually transmitted cancer. Isn't it strange that all these things we've been talking about are now coming out? Let me tell you about Peter Kawaja. He operated under a government code name. The reason he did this is that he found out that biological agents were being made in the United States, and he was aware of the fact that it was essentially treason, and that it was wrong, and he went to the CIA, FBI and U.S. Customs and told them that this was going on.

However, look what came out of the British press. On January 17, 1991. Let me show you how much information the British press had. Look at this, "Boca Plant begins trail to Washington." You see, what Peter Kawaja did is that he sent a FedEx letter to the NSA telling them terrorists were in the United States making biological weapons, and he wanted President Bush to know.

So, Peter Kawaja sent all the information to President Clinton on September 4, 1994. Clinton then started a "Presidential Advisory Task Force". I don't know if you have heard anything about this or not, but it is a whitewash and cover-up. It is incredible that they are making the American people think that they are really looking into the Gulf War illness.

UCSF finding suggests mechanism of experimental cancer drug, hinting at broader application for the drug, a modified cold virus

University of California San Francisco researchers may have discovered why the experimental anti-cancer drug Onyx-015 works more broadly than had been expected, a finding that could solidify and expand use of the drug and which reveals a potential new target for therapy that could prompt the development of other cancer drugs.

Onyx-015 is a genetically modified version of a cold virus that was designed to home in on cancer cells, specifically those in which the p53 tumor-suppressor gene - a key sentinel of cell health -- does not function. P53 is mutated and does not function in 60 percent of cancers. The theory has been that, without the p53 defense, Onyx-015 could replicate in cancer cells.

However, researchers have observed in clinical trials and cell culture studies that Onyx-015 also works against tumors in which the p53 tumor-suppressor gene appears to remain intact. This has created heated debate in the scientific community about the drug's efficacy and mechanism of action.

Now, the UCSF researchers believe they have solved the conundrum: In the October issue of Nature Medicine, they report that while some tumors responding to Onyx-015 have mutations in the p53 gene itself, others have a defect in another gene, known as p14 ARF, which is located "upstream" in the p53 pathway and which indirectly regulates p53 function. They showed that loss of p14 ARF leads to the deregulation of another protein, Mdm2, which in turn inhibits p53.

As a result, p53 cannot shut down the engineered virus's attempt to take over the cellular machinery, replicate incessantly and eventually kill the cell -- action that, counterintuitive as it may be, is just what researchers want to happen when it comes to cancer cells.

The finding "solves a big puzzle about the drug's mechanism of action," says a co-author of the study and the drug's creator, Frank McCormick, PhD, director of the UCSF Comprehensive Cancer Center, and co-founder of Onyx Pharmaceutical Inc., which develops the drug. "Inactivating p14ARF is just another way of turning off p53."

The finding also suggests the possibility of new applications for Onyx-015, says the senior author of the study, Michael Korn, MD, UCSF assistant adjunct professor of medicine. "Our work supports the idea of exploring the use of Onyx-015 in tumors that we presumed would be immune to the drug because they were thought to frequently have p53 genes, such as such as melanoma and glioblastoma [a form of brain cancer]. Clinical trials have to show if the p14 ARF and p53 status could serve as a reliable predictor of response to treatment with Onyx-015."

The finding could also lead researchers to consider how to design other genetically engineered viruses that take the p14 ARF mechanism into consideration, says Korn.

Moreover, he says, the discovery suggests that p14 ARF could potentially become a therapeutic target in itself. A drug that could reactivate p14 ARF would reactivate p53, thereby reinvigorating the cell's defenses.

To date, researchers have focused clinical trials of Onyx-015 on cancer types that previous studies have shown often have inactivated p53, though the drug has been administered randomly. The new findings, says Korn, will encourage the researchers to clarify the status of 53 and p14ARF in the individual tumors targeted.

In a recent multi-institutional phase II trial of Onyx-015 led by investigators at University of Texas M.D. Anderson Cancer Center, 30 patients with advanced head and neck cancer, most of whom had failed surgery or radiation therapy, were treated. The trial was designed to determine the appropriate dosage for the drug. In 63 percent of the patients, the tumors shrank by 50 percent or more, and in eight of these 19 responding patients the tumors disappeared altogether. (Nature Medicine, August 2000). The patients were treated with a combination of Onyx-015 and chemotherapy.

Because researchers later determined that the response to the combination therapy occurred both in tumors in which p53 was mutated and in which it was in tact, the finding left open the possibility that chemotherapy was the critical factor in the shrinking of tumors. However, says Korn, the new finding raises the possibility that the tumors in which p53 remained intact might have been defective in p14 ARF, and were, therefore, responding to Onyx-015. Based on this hypothesis, he says, "It will be important to assess the effect of Onyx-015 without chemotherapy, to see if the drug has the same potent effect."

According to Onyx Pharmaceuticals, researchers participating in ongoing phase II trials of Onyx-015 in different cancer types, including colorectal and pancreatic cancer, have observed a response to therapy in some patients. As is the case with all experimental cancer drugs, Onyx-015 still must be evaluated in a phase III trial, a much larger trial designed to test the efficacy of a drug in a large population. A phase III, multi-institutional clinical trial of Onyx-015 for head and neck cancer has begun.

In another study, Onyx Pharmaceuticals will evaluate the p53 status of tumors in patients who are participating in a phase II clinical trial in which Onyx-015 will be administered intravenously. To date, the drug has been administered in different ways: by direct injection into the tumor by installation into the peritoneal cavity in patients with ovarian cancer, and by injection into the artery that supports the liver in patients with gastrointestinal cancer metastatic to the liver.

Onyx-015 is an example of the new wave of cancer therapies emerging based on researchers' growing understanding of the molecular mechanisms of cancer and viruses. Researchers know that p53 plays the crucial role of halting cell division when it detects that a cell's DNA has been damaged, as occurs in cancer or when a virus enters a cell. In many cases, loss of p53 function is one of the first of several steps leading to the disruption of a cell's regulatory processes.

McCormick conceived the drug based on this knowledge of p53 - and the fact that when the adenovirus cold virus, a relatively harmless irritant, enters a cell, its first trick is to dismantle the p53 tumor suppressor gene, preventing it from signaling the cell to self-destruct in the face of an invader. Once the normal cold virus has taken over the cell's molecular machinery, it begins replicating, ultimately killing the cell. The adenovirus's victory leads to nothing more than sneezing and congestion.

But McCormick realized that if he disabled the protein within the cold virus that sabotages p53, the virus would not be able to replicate in normal cells, because p53 would remain intact and thwart the virus's efforts. The virus would, however, be able to grow in cancer cells in which the p53 gene was already disabled. In this way, the crippled adenovirus would discriminate between cancer cells and normal cells - a Holy Grail of cancer therapy.

In their effort to elucidate how Onyx-015 works in cells that appear to have normal p53 genes, the researchers examined, in culture, colorectal cancer cells that respond to Onyx-015 even though their p53 gene appears to remain in tact. They observed that when p14 ARF was mutated, p53 did not function, even though the p53 gene itself was not damaged. But when14 ARF was reintroduced in cells, p53 began functioning again and Onyx-015 was no longer able to replicate in the tumor cells. The explanation, they demonstrated, is that the loss of p14 ARF leads to the deregulation of another protein, Mdm2, which in turn inhibits p53. In this way, p53 is prevented from inducing the protective effects that would otherwise shut down the engineered virus. To confirm their findings, the researchers will examine tumor samples from cancer patients who have intact p53 genes but respond to Onyx-015, to see whether the p14 ARF pathway is, in fact, defective in these cells, says the lead author of the study, Stefan Ries, PhD, a postdoctoral fellow in McCormick' s lab.

In addition, researchers in McCormick's lab are attempting to create a mouse version of the Onyx-015 virus. If they succeed, they will then "knock out" either the p14ARF gene or the p53 gene in mouse tumors to see how the drug fares.

"We're definitely making progress in understanding Onyx-015," says Ries. "This was a very nice discovery."

Co-authors of the study are Christian H. Brandts, MD, a postdoctoral fellow, Alicia Chung, a staff research associate, Carola H. Biederer, PhD, a postdoctoral fellow, and Ettie M. Lipner, staff research associate, all in the McCormick laboratory, and Bryon C. Hann, MD, PhD, a specialist in the laboratory of Alan Balmain, PhD, UCSF professor of biochemistry.

The ongoing research is funded by Onyx Pharmaceuticals, Inc. (NASDAQ: Onxx). McCormick remains a paid scientific advisor to the company, and is a stockholder in the company. Korn is a paid clinical advisor to the company.

A U.S. Centers for Disease Control and Prevention report detailed cases of people ill in May of 2006 and from March to June of 2007 with a strain of the virus called adenovirus 14 in New York, Oregon, Washington state and Texas.

"Whether you're a healthy young adult, an infant or an elderly person, this virus can cause severe respiratory disease at any age," said John Su, who investigates infectious diseases for the CDC and contributed to the report.

"What makes this particular adenovirus a little different is that it has the capability of making healthy young adults severely ill. And that's unusual for an adenovirus, and that's why it's got our attention," Su said in a telephone interview.

Two of the 10 people who died from the new strain were infants, Su said. The CDC report said about 140 people were sickened by the virus and more than 50 hospitalized, including 24 admitted to intensive care units.

One of those who died was a 19-year-old female recruit at Lackland Air Force Base in Texas where other cases were found.

"Adenoviruses are notorious for causing illnesses, particularly in military recruits," said Dr. Anthony Fauci, director of the U.S. National Institute of Allergy and Infectious Diseases.

A CDC spokesman said there was no evidence the virus was currently causing disease anywhere in the United States.

Adenoviruses frequently cause acute upper respiratory tract infections like the common cold, but also can cause other illnesses including inflammation of the stomach and intestines, pink eye, bladder infection and rashes.

Colds caused by adenoviruses can be very severe in the very young and the very old as well as in certain other people, like those with compromised immune systems.

DIMENSION OF THE PROBLEM

Dr. William Schaffner, a spokesman for the National Foundation for Infectious Diseases, said an important next step is for public health officials to determine the dimension of the problem.

"I think this is a big alert to those of us in infectious diseases and public health to gather the appropriate specimens and see how widely distributed this virus is," said Schaffner, chairman of the Department of Preventive Medicine at Vanderbilt University School of Medicine in Nashville, Tennessee.

The first case described in the report was that of an infant girl in New York City who died in May 2006. Seven other people died in Oregon, including an infant. And a patient with AIDS died in Washington state.

Su said it was possible people outside the four states were sickened by the new strain of the virus.

"The cases described in this report are unusual because they suggest the emergence of a new and virulent Ad14 (adenovirus 14) variant that has spread within the United States," according to the CDC report.

There are 51 types of adenoviruses, the CDC report said.

Clinical features: Adenoviruses most commonly cause respiratory illness; however, depending on the infecting serotype, they may also cause various other illnesses, such as gastroenteritis, conjunctivitis, cystitis, and rash illness. Symptoms of respiratory illness caused by adenovirus infection range from the common cold syndrome to pneumonia, croup, and bronchitis. Patients with compromised immune systems are especially susceptible to severe complications of adenovirus infection. Acute respiratory disease (ARD), first recognized among military recruits during World War II, can be caused by adenovirus infections during conditions of crowding and stress.

The viruses: Adenoviruses are medium-sized (90-100 nm), nonenveloped icosohedral viruses containing double-stranded DNA. There are 49 immunologically distinct types (6 subgenera: A through F) that can cause human infections. Adenoviruses are unusually stable to chemical or physical agents and adverse pH conditions, allowing for prolonged survival outside of the body.

Epidemiologic features: Although epidemiologic characteristics of the adenoviruses vary by type, all are transmitted by direct contact, fecal-oral transmission, and occasionally waterborne transmission. Some types are capable of establishing persistent asymptomatic infections in tonsils, adenoids, and intestines of infected hosts, and shedding can occur for months or years. Some adenoviruses (e.g., serotypes 1, 2, 5, and 6) have been shown to be endemic in parts of the world where they have been studied, and infection is usually acquired during childhood. Other types cause sporadic infection and occasional outbreaks; for example, epidemic keratoconjunctivitis is associated with adenovirus serotypes 8, 19, and 37. Epidemics of febrile disease with conjunctivitis are associated with waterborne transmission of some adenovirus types, often centering around inadequately chlorinated swimming pools and small lakes. ARD is most often associated with adenovirus types 4 and 7 in the United States. Enteric adenoviruses 40 and 41 cause gastroenteritis, usually in children. For some adenovirus serotypes, the clinical spectrum of disease associated with infection varies depending on the site of infection; for example, infection with adenovirus 7 acquired by inhalation is associated with severe lower respiratory tract disease, whereas oral transmission of the virus typically causes no or mild disease. Outbreaks of adenovirus-associated respiratory disease have been more common in the late winter, spring, and early summer; however, adenovirus infections can occur throughout the year.

Diagnosis: Antigen detection, polymerase chain reaction assay, virus isolation, and serology can be used to identify adenovirus infections. Adenovirus typing is usually accomplished by hemagglutination-inhibition and/or neutralization with type-specific antisera. Since adenovirus can be excreted for prolonged periods, the presence of virus does not necessarily mean it is associated with disease.

Treatment: Most infections are mild and require no therapy or only symptomatic treatment. Because there is no virus-specific therapy, serious adenovirus illness can be managed only by treating symptoms and complications of the infection.

Prevention: Vaccines were developed for adenovirus serotypes 4 and 7, but were available only for preventing ARD among military recruits. Strict attention to good infection-control practices is effective for stopping nosocomial outbreaks of adenovirus-associated disease, such as epidemic keratoconjunctivitis. Maintaining adequate levels of chlorination is necessary for preventing swimming pool-associated outbreaks of adenovirus conjunctivitis.

Scores sickened as mutated virus becoming more common, CDC says

ATLANTA - A mutated version of a common cold virus has caused 10 deaths in the last 18 months, U.S. health officials said Thursday.

Adenoviruses usually cause respiratory infections that aren’t considered lethal. But a new variant has caused at least 140 illnesses in New York, Oregon, Washington and Texas, according to a report issued Thursday by the U.S. Centers for Disease Control and Prevention.

CDC officials don’t consider the mutation to be a cause for alarm for most people, and they’re not recommending any new precautions for the general public.

“It’s an uncommon infection,” said Dr. Larry Anderson, a CDC epidemiologist.

The illness made headlines in Texas earlier this year, when a so-called boot camp flu sickened hundreds at Lackland Air Force Base in San Antonio. The most serious cases were blamed on the emerging virus and one 19-year-old trainee died.

“What really got people’s attention is these are healthy young adults landing in the hospital and, in some cases, the ICU,” said Dr. John Su, an infectious diseases investigator with the CDC.

There are more than 50 distinct types of adenoviruses tied to human illnesses. They are one cause of the common cold, and also trigger pneumonia and bronchitis. Severe illnesses are more likely in people with weaker immune systems.

Some adenoviruses have also been blamed for gastroenteritis, conjunctivitis and cystitis.

There are no good antiviral medications for adenoviruses. Patients usually are treated with aspirin, liquids and bed rest.

Some people who get infected by the new bug probably would not suffer symptoms, and some may just feel a common cold. Sick people should see a doctor if they suffer a high fever or have trouble breathing, Anderson said.

In the CDC report, the earliest case of the mutated virus was found in an infant girl in New York City, who died last year. The child seemed healthy right after birth, but then became dehydrated and lost appetite. She died 12 days after she was born.

Tests found that she been infected with a form of adenovirus, called Ad14, but with some little differences, Su said.

More common
It’s not clear how the changes made it more lethal, said Linda Gooding, an Emory University researcher who specializes in adenoviruses.

Earlier this year, hundreds of trainees at Lackland became ill with respiratory infections. Tests showed a variety of adenoviruses in the trainees, but at least 106 — and probably more — had the mutated form of Ad14, including five who ended up in an intensive care unit

In April, Oregon health officials learned of a cluster of cases at a Portland-area hospital. They ultimately counted 31 cases, including seven who died with severe pneumonia. The next month, Washington state officials reported four hospitalized patients had the same mutated virus. One, who also had AIDS, died.

The Ad14 form of adenovirus was first identified in 1955. In 1969, it was blamed for a rash of illnesses in military recruits stationed in Europe, but it’s been detected rarely since then. But it seems to growing more common.

The strain accounted for 6 percent of adenovirus samples collected in 22 medical facilities in 2006, while none was seen the previous two years, according to a study published this month in the medical journal Clinical Infectious Diseases.

The new bug could have implications for the military. Other forms of adenoviruses have been a common cause of illness in recruits. Military officials are bringing back an adenovirus vaccine — administered as a pill — that was given to recruits from 1971 to 1999, CDC officials said.

A Barr Pharmaceuticals vaccine for the military, currently being tested, is expected to be licensed in 2009. Like the old pill, it focuses on adenovirus serotypes 4 and 7, because those bugs have been persistent problems, said Col. Art Brown, an Army physician involved in the product’s development.

Some CDC officials said a vaccination against the mutant Ad14 might be needed. Brown said it isn’t clear if the mutant Ad14 will be an enduring threat, but the military will monitor illness reports.

“If it persists, then we’d consider if the vaccine needs to be modified further,” said Brown, of the U.S. Army Medical Materiel Development Activity.

A Maryland school district has ordered parents of more than 2,300 students to court Saturday for failure to immunize their children. The parents could face fines and jail time if they do not appear.

The students, in grades five through 10, will be required to report to court with their parents. While students can expect to be vaccinated before they leave the courthouse, parents will be lectured to by Circuit Court Judge Philip Nichols Jr. on the necessity of vaccination. Those who fail to appear face fines of $50 a day and up to 10 days in jail.

According to Prince George's County Schools spokesman John White, the students were required to receive two shots before Sept. 20: one for hepatitis B and the other for the chickenpox. Parents who did not meet the deadline for both shots were asked not to bring their children to school until they were immunized or provided proof of an immunization appointment.

"This shows you how redundant our attempts have been. In many cases kids still have to come to school, and when they come to pick their child up, they are asked if they could sign the consent, and in some cases they don't sign it," said White.

According to Prince George's County Schools, neglecting to have a child vaccinated is not the norm. Out of 132,000 students in the county school system, 2,300 students have been tardy in meeting the immunization deadline. "The majority of parents have done the right thing," said White.

Despite discrepancies in state immunization laws, the nationwide vaccination rate is approximately 95 percent, according to the Centers for Disease Control and Prevention.
